Performance Grading System from the Wilson-Oyler Group

While filling out the Performance Grading report card you will have the opportunity to enter your Action Plan by answering three questions for each score of three or below. The Action Plan consists of three questions : Action to take, Who will complete the Action and When/By the action will be completed.
